Billing

  • Your program is billed on a 28-day cycle, every four (4) weeks.
  • Billing continues even if you travel, don't use a visit, or are inactive — your subscription doesn't pause automatically. Let us know if you'll be late due to an unforeseen emergency (illness, travel, etc). If you need to travel or take a break, contact us to plan around it.
  • Your minimum commitment is 180 days. After that, it continues month-to-month.
  • All fees are non-refundable — including the $147 assessment fee, which is not applied toward program cost even if you continue.

If You're on the Insurance Track

  • You're responsible for returning your member benefits interview packet. We cannot proceed with your prior authorization without it.
  • We submit prior authorization and advocate on your behalf — but we cannot guarantee your insurance will approve or cover treatment. Coverage decisions are made by your insurer, not by us. You're responsible for keeping your insurance active and providing any denial letters you receive.
  • We do not accept Medicare, Medicaid/AHCCCS, Tricare, or any government insurance. If you have government insurance, you'll need to choose a cash-pay program instead.
  • If your prior authorization is denied, you'll have options: pay cash or your copay to your pharmacy, or move to a cash-pay program — your choice; we support that choice.

Medication Pickup (Cash-Pay Programs)

Testosterone, Nandrolone, Phentermine, and Phendimetrazine are federally controlled substances. By law, they must be picked up in person, every 28 days — no mail, no designee pickup, no exceptions. Plus, we like seeing your face.

Program Changes

  • You can switch programs up to 2 times per calendar year.
  • For some weight loss medications, your dose restarts rather than carrying over — this is a safety requirement, not a penalty.

Weight Loss Controlled Substance Cycles

  • Phentermine and phendimetrazine cycles run 3 cycles, followed by a mandatory 2-week wean-down and a 1-cycle break before the next cycle. This is a clinical safety standard, not a delay tactic.
  • To remain on controlled substances, we require a minimum 1 lb/week loss. If you're not hitting that pace, your physician will adjust your plan with you.

HRT Controlled Substance Cycles

Nandrolone cycles run 3 cycles, followed by a mandatory 1-cycle break before the next cycle. This is a clinical safety standard, not a delay tactic.

Add-Ons

Some add-on treatments require additional labs or clinical indication before we can approve them. If something isn't available to you right away, it's because we're making sure it's safe and appropriate — not because we're trying to upsell you.
