All programs bill every four (4) weeks with a 180-day minimum commitment.

Insurance vs. Cash

Arizona Endocrinology is a hybrid clinic — patients choose the insurance track or cash-pay track during the assessment visit.

Add-On Treatments

Add-ons may be added anytime, subject to physician approval and prescription. Add-ons don't restart your 180-day term and begin at your next visit.

Switching Programs

Switches may require up to 60 days' notice and are limited to 2 per calendar year. Each switch restarts a new 180-day term; the $147 assessment fee is waived. Switching from cash-pay to the insurance-based Male Hormone track requires a 3-week washout and repeat labs (membership fee still applies during washout if you have not yet completed the 180-day commitment). New programs begin at the next billing cycle. Denied or delayed prior authorizations require a new agreement to switch.

Card on File

A wet-signed card authorization is required for every patient. Financial data is protected under our internal Financial Security SOP; access is strictly limited.

Payment Timing

Payment must be fully processed before medication is dispensed. See your Program Subscription Agreement for your payment schedule. Payments are processed on Mondays.

Assessment Fee

$147, separate from program fees, non-refundable regardless of outcome.

Laboratory Charges

Choose in-office cash-pay labs (flat fee) or your own insurance lab, with results submitted as a downloaded physician-view PDF (no screenshots). Lab review is included in your visit.

Medication Costs

Cash-pay medications are included in your program price and become your property once dispensed — non-refundable, non-returnable. Practice preferred-pharmacy medications are billed exact pharmacy cost + 3%; insurance-track costs depend on coverage.

Medication Switching

Within your tier, you may switch medications or protocols at the same price point (e.g., during a required break) at no extra cost.

Declined Payment

A declined card suspends visits, refills, and medications until resolved; the remaining 180-day balance may still be pursued.

Refunds

All payments are final. No exceptions.

Early Termination

No termination fee — you're only responsible for program cost through the remainder of your 180-day term.

Price Guarantee

Your price is locked at enrollment for the life of your agreement.
