Where are you located? 2600 E Southern Ave, Building G, Tempe AZ 85282.  We are inside Southern Desert Medical Center, which is located a quarter of a mile east of the Loop 101 and west of Dobson Road on the north side of Southern Ave. Once in the parking lot, look for the mail box and park. Then as you walk between building F and building H you will see a G on the building in front of you. That is our building. To get to the front door, walk around the building by turning left and then turn right when you see the large yellow geometric sculpture. The front door will be to your right as you approach the main street (Southern Ave).

Do I need to bring anything with me? Yes — a driver's license or state ID, and your insurance card if you're using insurance.

Can I switch programs later? Yes, up to two times a year.

​

Do I need a referral to come in? Nope. Just walk in.

​

Is there ever a wait? We move as quickly as we can. Because we're walk-in, there may be other patients ahead of you, but we do our best to see everyone quickly.  We do offer VIP programs; they never wait.

Can someone come with me to my visit? Yes. We love meeting new people.
